According to reports, he ignored a rule that required all guns, whether real or fake, to be announced on set to ensure safety procedures are adhered to.

Hall was also accused in shortening gun safety meetings and remarking that it seemed pointless since the same procedures were followed for all motive sets.

On Saturday, sources told the Daily Beast that Hannah Gutierrez-Reed, the 24-year-old armorer on set, was ‘careless’ and had previously handed a gun to an 11-year-old actress on a different set.

According to one source, Gutierrez-Reed worked with Gutierrez on The Old Way, which will feature Nicolas Cage.

Hutchins’ friend, and colleague Director Bandar Albaliwi started the petition. He wrote: ‘We must ensure that this avoidable tragedy does not happen again. There is no excuse that something like this could happen in the 21st Century. On film production sets, real guns are not necessary.

He tweeted: ‘There’s no reason to have guns loaded with blanks or anything on set anymore. Should be completely outlawed. There’s computers now.

It is not known who loaded the weapon and why it went off as a replacement crew was brought in the day of the incident (The gun pictured above is a vintage Colt pistol manufactured between 1873-92.  The gun that killed the cinematographer on the set of Alec Baldwin’s Rust had been used for target practice by crew members, sources linked to the western film’s production said.

Multiple sources close to the Rust set told TMZ that crew members had used the Colt pistol recreationally before it went off in Alec Baldwin’s hands, killing Halyna Houtchins and injuring Joel Souza.

Sources claim that crew members would practice target shooting with real bullets. Others believe that a live round from practice sessions made it onto the set.

Another source said to TMZ that live ammunition and blanks were stored in the same area as the set. This could be another explanation for how a bullet was fired out of Baldwin’s Colt.

A warrant was released Friday stating that Hannah Gutierrez-Reed (24), a Rust armorer, had placed three prop guns on a cart near the filming location. First assistant director Dave Halls then grabbed the Colt from that cart and brought it into Baldwin, not realizing that it was loaded up with live rounds.

“Cold gun!” Halls shouted, “Cold gun!” before handing Baldwin the gun. The phrase was used to signal to the cast and crew that it was safe to fire at the scene, according to the warrant.

Baldwin, who was filming an Old West-style scene inside a church, apparently pointed towards the camera and pulled out the trigger. She reportedly accidentally killed Hutchins while filming him and injures Souza, who stood by her.

Two production sources familiar with Gutierrez Reed’s work said that it wasn’t the first time she had been involved in an incident at a movie set.

Two sources informed The Daily Beast that Hannah Gutierrez–Reed allegedly gave an 11-year old actress a gun, without properly inspecting it, while she was filming The Old Way with Nicholas Cage.

One source said that she loaded the blanks a couple of times and did it in a way that was unsafe.

“She was a bit careless about the guns, waving them around every now-and-then.”

Sources on the Rust team have stated that the fatal incident which killed Hutchins, 42 years old, and injured Souza 48 years ago was caused by production problems from top to bottom.

They also stated that Halls, the assistant director, handed Baldwin the gun and said it was safe.

One source on set stated that ‘He’s supposed be our last line and he failed us’. “He’s not supposed to be looking at that firearm.”

According to a Rust production source, there have been at least two other instances of guns accidentally being discharged by crew members on set prior to Thursday’s tragic incident.

GutierrezReed was described as ‘inexperienced’ and ‘green’ by the source.

Hutchins, who was 13 years old, died in Albuquerque’s University of New Mexico Hospital following a fatal accident on Thursday while filming ‘Rust’, a western about a boy running away from his grandfather, played by Baldwin.

Matthew Hutchins tweeted Friday’s photo of his wife and their nine year-old son Andros. The caption read: “Halyna instigated us all with her passion, vision, and her legacy too meaningful to be encapsulated in words.

“Our loss was immense. We ask that the media respect my family’s privacy during our time of grief. We are grateful to everyone who shared images and stories about her life.

DailyMail.com’s husband, who was in shock, said that he had spoken to the actor on Friday morning. Alec Baldwin has been supportive of my family, he stated.

Baldwin shared via social media that he was shocked and sad by the accident that resulted in Hutchin’s passing and left Joel Souza with injuries.

Baldwin posted via Twitter that he couldn’t express his shock and sadness at the tragic accident that claimed the life of Halyna HUTCHINS, a wife and mother and a colleague who was deeply admired.

He also stated that he was fully cooperating in the police investigation. IATSE Local 44, which is a labor union for professionals working in the entertainment industry, stated that Baldwin used a prop gun to fire a single road.

The film crew admitted that they left the set hours before the fatal accident because of safety concerns. Three firearms were accidentally discharged, including one by Baldwin’s stunt double, who was told that the gun was not loaded. They also accidentally discharged the gun twice in a closed-cabin.

Friday’s search warrant stated that Hannah Gutierrez Reed set up three prop guns outside the filming location. Dave Halls, the first assistant director, grabbed the gun and brought it inside. However, he was not aware that it was loaded with live ammunition.

“Cold gun!” Halls shouted “Cold gun!” Before handing the gun to Baldwin. This phrase was used to signal the crew and cast that the gun was safe to be fired for the scene, the warrant stated.

Baldwin, 62, was filming scenes for Rust, when the gun went off. Hutchins died, and Joel Souza (48), was left with injuries. The incident occurred at Bonanza Creek Ranch.

Hutchins was airlifted to the University of New Mexico Hospital by an air ambulance, but was declared dead shortly afterwards.

Souza was taken by ambulance from his home to the Christus St Vincent Regional Medical Center. He was released, but his condition is not known.

Will Stewart, Daily Mail’s Moscow correspondent told how Halyna had been for him in mid-2000s.

“This is shocking and unintelligible news about Halyna’s death. After graduating from a local university, she was trained in International Journalism and worked for my news agency from Kyiv during the mid-2000s.

‘Halyna was involved as a journalist in many British magazines and newspapers. But she also had a talent for documentaries. This was her first exposure to filmmaking, and it was this that she became so successful in America.

“At this point while she was with me, in 2006, she was Associate producer on a Discovery Channel documentary about Ukrainian icon Leonid Statnyk, called World’s Tallest Man, made British company Wild Pictures.

‘She was instrumental to Mr Stadnyk feeling at home participating in the documentary, which focused on the problems associated with being extraordinarily tall.

“The film, which was directed by Richard Denton in Britain, was produced with Paul Hamann, the former Head of Documentaries BBC, as executive producers.

‘Mr Stadnyk is a shy man who trusted Halyna, after refusing many other film offers. He thanked her later.

‘Pictures in advance of the film show him towering over Halyna’s home in the village Podoliantsy.

“She later went to America with her husband Matt. They achieved great success through her immense talent and creativity as well as her constant determination to learn.

“She once told her Russian colleagues that she felt that she was a perpetual student in America. But her efforts paid off, and she finally achieved the success she so deserved.

“It is tragic that she died in such an inexplicable and cruel way while doing the job she loved.

“Our thoughts, prayers, and best wishes to Matt and their son, Halyna and their family in Ukraine.

Richard Denton, the producer of Shakespeare Uncovered and many films from the former USSR, stated today that Halyna was the most wonderful person to work alongside.

“She was friendly and extremely helpful. She was able to translate interviews and make Leonid’s horse go in the right direction.

“She was completely unpretentious, and incredibly professional.”

Her death was’senseless, stupid’
